Monitor, Evaluate, Assess/Address, Treat — an AAPC / CDI mnemonic. Useful as a heuristic. Not a CMS regulation.

Documentation support against ICD-10-CM Official Guidelines, AHA Coding Clinic, and MMCM Ch. 7. Inpatient chronic conditions follow the AHA / UHDDS exception. MEAT stays ready_not_active.
